General Information about You
UUFSB staff and lay leaders sometimes need to reach out to people with specific expertise and skills. Please let us know a little bit about your current and/or former work experience.
This information is only available to UUFSB staff but could be shared with first responders or other medical professionals in the event of a medical emergency at UUFSB.
If you would like to do so, please write a short introduction of yourself that will be available to staff and other UUFSB members.
UUFSB depends on the financial generosity of its members to fulfill its mission. Every member is asked to make a pledge to the congregation. Pledges are considered to be on-going until UUFSB is notified that a member wants to increase, decrease, or stop their pledge.
However, UUFSB never wants financial difficulty to be an obstacle to membership, so financial exemptions for pledging are available for any individual experiencing extreme financial difficulty.
Members are asked to make a pledge based on a percentage of their income. Use the chart below to help decide an appropriate pledge for you.

UUFSB has a online and printed directory that includes pictures, names, and contact information. It is only available to staff and other UUFSB members. Please indicate below whether you would like your picture, name, and contact information to be included in the directory. If applicable, please indicate whether you would like your children's names included in the directory.
UUFSB takes pictures and videos of congregational events and uses these on its website, on social media, and in printed promotional materials. Please indicate whether you give your permission for you image to be used in this way. If applicable, please indicate whether you give permission for images of any minor children to be used in this way.
Please provide a photo of yourself, or, if applicable, of your family. This will help us to get to know you better, and if you have given permission above, will be included in our directory.
